Developing and using quality indicators in French health care organisations: a new area of health services and management research, Lessons from the Compaq-Hpst project

Abstract

Quality of care is measured in many countries. However, using quality indicators (QIs) to assess the performance of health care organizations (HCO) is a new development in the management and health services research fields because national quality assessment initiatives are relatively recent. In the United States, that is well advanced in the field of quality measurement, the first initiatives including the Hedis project, the Joint Commission ORYX initiative, Hospital Compare, and the development of the Agency for Healthcare Research and Quality date back to the nineties. The Australian Council on Health Care Standards, the Ontario Hospitals Association in Canada, and the Clinical Indicators Support Teams of Scotland were launched just before 2000. The Dutch, UK and German programs are more recent. Likewise, the Performance Assessment Tool for Quality Improvement in Hospital (PATH) supported by the World Health Organization, and the Organization for Economic Co-operation and Development (OECD) initiatives were developed during the last decade. Scientific research programs appeared over the same period in many countries, albeit with specificities that took into account the differences in each health care system including the choice of public health priorities, the development of the health information system, and the direction taken by the quality of care policies. Substantial advancement in the field of quality and safety that was brought by those programs has slowly integrated QIs research to the field of health services research. However, in the management science field, important knowledge gaps remain in the area of QIs utilization for assessing and improving performance in health care. The Aim : of this article is to describe this integration process in health services and management research and its requirement and to shed light on QIs research perspectives in the field of health care performance improvement. We will use the example of a French QIs research program as a framework.
