Évaluation du plan d’amélioration de la prise en charge de la douleur 2006–2010

Résumé

The High Council of Public health was commandeered by the Chief Executive Officer of the Care Organization to evaluate the third national plan of improvement of the care of pain. A multidisciplinary committee gathered skills in the domains of public health, evaluation and various disciplines concerned by pain. It relied on the analysis of documents, including written contributions collected on the basis of specifications and auditions. The evaluation contained a balance sheet of the actions realized in every priority of the plan, an appraisal of the relevance of the objectives and governance and recommendations for the future. The third plan had an impact on the professional environment of the hospital in terms of animation, encouragement for innovation, recommendations, quality requirement for care; the prevention of care-induced acute pains in children; the training for nursing old persons. However, this plan did not take enough into account exogenous constraints such as the organization and the financing of the public hospital, the relative isolation of the liberal medicine and the conditions of salary imposed by the health insurance on both the medical and non-medical professionals. In spite of undeniable progress, public authorities must contribute further to a better care of pain. The analysis of the actors and actions engaged in the plan allows to point out weak points and to propose new perspectives for the realization of a fourth plan.
Le Haut Conseil de la santé publique (HCSP) a été saisi par la directrice générale de l’offre de soins (DGOS) en vue d’évaluer le troisième plan national d’amélioration de la prise en charge de la douleur. Un comité pluridisciplinaire rassemblant des compétences dans les domaines de la santé publique et de l’évaluation et, en matière de douleur, dans les différentes disciplines concernées, s’est appuyé sur l’analyse de documents, des auditions et des contributions écrites recueillies sur la base d’un cahier des charges. L’évaluation a comporté un bilan des actions réalisées dans chaque priorité du plan ; un jugement sur la pertinence de ses objectifs et sa gouvernance ; et la formulation de recommandations pour l’avenir. Le troisième plan a eu un impact sur le milieu professionnel hospitalier en termes d’animation ou d’encouragement à l’innovation, en matière de recommandations, d’exigence de qualité de la prise en charge ; dans la prévention des douleurs aiguës liées aux soins chez l’enfant ; pour la formation des soignants en charge des personnes âgées. Toutefois, ce plan n’a pas suffisamment pris en compte les contraintes exogènes que sont l’organisation et le financement de l’hôpital public ; le relatif isolement de la médecine libérale ; et les conditions de rémunération imposées par l’assurance maladie aux professionnels médicaux et non médicaux. Le comité de suivi a peu assuré son rôle. Malgré d’indéniables avancées, les pouvoirs publics doivent encore contribuer à une meilleure prise en charge de la douleur. L’analyse des actions et des acteurs engagés dans le plan permet de repérer les points faibles, et de proposer de nouvelles perspectives pour la réalisation d’un quatrième plan.
